HEALTH AND WELLNESS
Positive Transitioning’s Health and Wellness program will help those individuals that are suffering with addiction or a medical condition to learn how to manage this on their own. This will be done by providing services for alcohol and substance abuse that can assist in getting and staying sober. Those that need medical management will be assisted in obtaining health insurance, finding the appropriate Dr. to manage their medical conditions as well as making the Dr. appointments. Positive Transitions Health and Wellness program will also assist in arranging transportation to these appointments as well as to pick up medications.This will allow for the individual to learn to manage their own medical conditions as well as assist them in obtaining any medications that need to be continued after incarceration.

Reentry bags
Positive Transitioning is actively seeking funding for our reentry bags program. Rejoining the community after leaving prison is difficult in itself: ex-offenders face difficulty finding housing, employment opportunities, and reintegrating into their communities. We hope to give people the helping hand they need in the form of reentry bags. These bags will include food items conducive to a balanced diet, as well as recipes and tips for healthy eating. Individuals often develop diabetes, high blood pressure and other cardiovascular illnesses while in prison that affect their overall wellbeing throughout their lives. Positive Transitioning is committed to bettering the lives of ex-offenders by giving them the tools they need to reenter their communities with success.
Bags will include one month of groceries and a meal plan that includes 30 days of possible recipes. Meal plans will be individualized to the person’s dietary restrictions, taking into consideration religious restrictions and lifestyle choices. The bags will contain $150 worth of food, as well as clothes, personal hygiene items, and a smartphone if needed. This is a nationwide service, and bags will be delivered to individuals who are nearing their release dates.
